CCALAC represents non-profit community clinics that operate primary care sites throughout the county. Our 66 member organizations operate over 300 sites throughout Los Angeles and serve as the medical home for nearly 1.5 million patients each year. Community clinics provide primary health care, including medical, dental, and mental health services to the uninsured, under-insured, low-income, high-risk and vulnerable populations. They serve all, regardless of ability to pay. CCALAC’s central role is to help our members serve their patients in an efficient and cost-effective manner while providing quality care. The association strives to identify and address the collective needs of our members at the local, state and federal levels. CCALAC delivers a variety of member services including education, and peer support. We connect clinics, share and leverage resources, increase organizational capacity, and raise a unified voice on behalf of clinics. Previously known as the HealthWorks LA AmeriCorps, the CCALAC AmeriCorps Health Fellows program is CCALAC’s AmeriCorps program focused specifically community health and providing access to primary care and health services to LA County’s underserved. Participants in the program are referred to as AmeriCorps Health Fellows. Full-Time AmeriCorps Health Fellows serve 1700 hours (35-40 hours a week) at a CCALAC member clinic in exchange for a living allowance, education award, work experience, and the opportunity to give back to the community. AmeriCorps Health Fellows do not provide health services related to the prohibited activities (including abortion referrals or services)

Member Duties : 1) Outreach: Conduct outreach to new/existing patients at: health fairs, community organizations, waiting/exam rooms, universities, and community events to inform/educate about primary care services. 2) Enrollment: Assist patients with the enrollment process into health programs, health insurance plans and/or pharmaceutical assistance programs. This includes MediCal and MHLA. 3) Case Management: Provide support for patients in navigating health systems, service and programs. This includes assisting patients with the Patient Portal, translation, program referrals, housing and food security, and other social determinants of health. 4) Health Education: Provide linguistically and culturally appropriate health education in one-on-one or group setting. Engage and educate patients to improve health outcomes in chronic disease control. AmeriCorps Health Fellows do not provide health services related to the prohibited activities, including abortion referrals or services.
